RESTAURANT INSPECTONS: Nov. 12, 2012

TERRE HAUTE — The following food establishments were inspected Oct. 22-26.

La Tienda de Maria, 5127 S. U.S. 41 (6 Critical, 7 Non-Critical) Pork chorizo, queso cheese, raw pork, ham lunch meat, raw eggs found at 49-74 degrees; should be 41 degrees or less; handwash sink in kitchen blocked by chair; observed employee handling/preparing tacos with bare hands; employee not sanitizing dishes after washing; beans, pork and chicken not marked with date of consumption (discarded); found rotten limes in cooler in kitchen and rotten watermelon on floor in storage room.

Crazy Buffett, 4676 S. U.S. 41 (2 Critical, 4 Non-Critical) Food on hot bar held at 108-118 degrees; should be 135 degrees or more; cooked food in walk-in cooler not properly date marked.

Pizza Hut of Vigo County, 3040 Wabash Ave. (2 Critical, 3 Non-Critical) Front handwash sink found dirty; black debris found on soda nozzles and inside ice machine.

Burger King No. 11099, 2575 Indiana 46 (1 Critical, 5 Non-Critical) Debris found on ice machine chute.

Applebee's Neighborhood Grill, 1950 Fort Harrison Road (1 Critical, 5 Non-Critical) Burned-on debris observed accumulated in chips drawer.

Sonic Drive-In, 2149 S. Indiana 46 (1 Critical, 2 Non-Critical) Cooking utensil not washed, rinsed and sanitized every four hours.

Coffee Cup Family Restaurant, 1512 Lafayette Ave. (1 Critical, 2 Non-Critical) Soup, cooked pasta, rice pilaf not date marked.

Bayles Marathon, 3000 U.S. 41 S. (1 Critical, 1 Non-Critical) Spray nozzle below flood rim at 3-bay sink, no air gap provided to prevent back siphonage.

Bethany House, 1402 Locust St. (1 Critical) Ice observed in handwash sink.
