Indiana Public Health Foundation seeks nominations

TERRE HAUTE — The Indiana Public Health Foundation Inc. is now planning its 2012 “Tony and Mary Hulman Health Achievement Awards” gala for Nov. 3 in the Westin Hotel at Capitol Avenue and Washington Street in Indianapolis. This is the foundation’s 28th year to recognize outstanding contributions of health professionals and organizations. The event will begin with a reception, followed by dinner, the awards program, and end with an evening of dancing.

Indiana’s Premier Health Awards will be presented in these categories: Environmental Health, Geriatrics and Gerontology, and Preventive Medicine and Public Health. These awards are presented to individuals and to businesses and industries or organizations that make significant contributions in these fields. The foundation also presents an award for Excellence in Health Science Research and the Helen L. Scheibner Scholarship to a qualified student in the field of public health. The Foundation board of directors selects an outstanding individual for the Lifetime Award for Distinguished Service in Years of Health Advancement.

Organizers are now seeking nominations for the awards. In making a nomination, consideration should be given to people whose dedication and exceptional services have made a significant change or impact on the human state, scientific advances, or improved environmental health conditions. Criteria and forms for nominating individuals, business, industry or organizations are available at: www.iphf.us. Forms also may be emailed upon request.

Nominators should provide a list of those who know the nominee and who would want to attend the dinner in his or her honor. A table for eight is $750, or individual tickets are $100 each. It is also meaningful to the recipient if the person or organization that nominates the individual or business identifies someone to present that award to the person. More information about attending the awards dinner will be sent once the award winners are selected. Proceeds from the dinner are used to support the Helen L. Scheibner Scholarship for Life Sciences and Public Health, the Health Science Research monetary award, for other health related activities and general operating expenses.

Nominations will be reviewed by an awards selection committee. Nominations may be submitted by mail to Awards Committee, Indiana Public Health Foundation Inc., 8063 Madison Avenue, PMB 289, Indianapolis, IN 46227, no later than Aug. 31. The nominations may be faxed to 317-889-0399, or emailed to the executive director at helenscheibner@netscape.net. Recipients will be notified by Sept. 7, as will the nominating body or person.
