RESTAURANT INSPECTIONS: April 16, 2012

TERRE HAUTE — March 26-30:

La Tienda De Maria, 5127 S. U.S. 41 (4 Critical, 4 Non-Critical). Found raw pork stored with cooked pork chops in meat case; handwash sink found dirty; red and green sauce and cooked pork not marked with date of consumption; Fried pork found packaged and stored at room temperature — should be 41 degrees or less (discarded). Establishment closed until reinspection on March 29.

Garfield’s Restaurant & Pub, 3401 S. U.S. 41, Suite D-7 (3 Critical, 5 Non-Critical). Raw steaks, chicken and shrimp in drawers below grill found at 52 degrees (discarded) and cooked ribs found at 50 degrees — should be 41 degrees or less; sliced deli meats, shredded cheese, mashed potatoes found in toppings cooler at 48-51 degrees, baked potatoes on tray found at 89 degrees (discarded) — should be 41 degrees or less; ice chute found with pink debris, old food debris found in containers in walk-in, slicer and chopper unclean.

MDT Corp. Sun-Mart, 408 S. Seventh St. (2 Critical, 4 Non-Critical). No sanitizer provided; observed slimy debris on ice machine chute.

Terminal Sports & Spirits, 820 Wabash Ave. (2 Critical, 1 Non-Critical). No sanitizer measured in dishwasher; found rotten celery in fridge.

McDonald’s, 229 S. Third St. (1 Critical, 3 Non-Critical). No sanitizer measured in wiping cloth bucket.

Ole Simrells, 804 S. Seventh St. (1 Critical, 3 Non-Critical). Certified Food Handler expired.

Circle K, 6270 E. Wabash Ave. (1 Critical, 2 Non-Critical). Found black debris on soda fountain nozzles and froster nozzles.

Umi Grill Sushi Bar, 2002 S. Third St. (1 Critical, 2 Non-Critical). Ice observed in handwash sink in grill line.

Arby's No. 7082, 2155 Lafayette Ave. (1 Critical, 2 Non-Critical). Raw roast beef in bag stored on top of lettuce (corrected).

Idle Creek Golf Course Inc., 5353 Eldredge Road (1 Critical, 1 Non-Critical) Soda Fountain nozzle observed with mold.

The Coffee Grounds, 423 Wabash Ave, (1 Critical, 1 Non-Critical). Found pink debris on ice machine chute.

Hilton Garden Inn, 750 Wabash Ave. (1 Critical, 1 Non-Critical). Hard-boiled eggs and bean and corn relish held past date of disposal (discarded).

Denny’s Restaurant No. 392, 3442 U.S. 41 (1 Critical, 1 Non-Critical). Wiping cloth buckets at waitress station and at front line found without sanitizer.

Square Donuts, 935 Wabash Ave. (1 Critical). Observed employee icing doughnuts with bare hands.

Cinnabon No. 235, 3401 S. U.S. 41 (1 Critical). Pink debris observed on ice chute in ice machine.
