Third St. (4 Critical, 1 Non-Critical) Employee observed assembling sandwich without wearing gloves; eggs found left out by grill at 73 degrees (discarded); scalloped potatoes, pulled pork, corn, gravy and sauce not marked with date of disposal; no sanitizer measured in dishwash machine.

Biryani Bowl LLC DBA Persis, 2309 S. Third St. (2 Critical, 4 Non-Critical) Pitcher of mango juice/water found with mold (discarded); sauce in small cooler in wait station 53 degrees.

Rally’s No. 9102, 3175 Wabash Ave., (1 Critical, 2 Non-Critical) No proof of certified food handler on staff.

Stables Steakhouse, 939 Poplar St. (1 Critical, 2 Non-Critical)  Accumulated debris found in container for ice machine ice scoop and bar soda nozzle holder at upstairs bar.

Sparks Stop 5 Tavern, 1915-17 Lee Ave. (1 Critical, 1 Non-Critical) Bar soda nozzle found with debris.
