McDonald’s, 2633 S. St. Rd. 46, (6 Critical, 3 Non-Critical) – Ribs founds at 123 degrees should be 135 degrees or above (discarded); Observed black debris inside of ice machine, on soda fountain nozzles in lobby and drive thru, dirty dishes found on clean dish rack; Warmer racks found with accumulated grease and debris, dish rack for clean dishes found with accumulated debris; Observed drain leaking at front handwash sink and 3-bay sink; Found back handwash sink dirty; Observed spray bottles of cleaner above prep table and single service items.

1st Wok, 2570 Wabash Ave., (4 Critical, 2 Non-Critical) – Cooked rice observed on floor, fried chicken on bottom shelf of prep table, not covered; 2 large woks with oil and food debris observed stacked on dirty floor mats on shelf; Inside of ice machine observed with mold; Bowls and utensils rinsed in cold water, not washed, rinsed and sanitized.

Domino’s Pizza, 1330 Wabash Ave., (3 Critical, 2 Non-Critical) – Observed spray nozzle at 3-bay sink hanging below flood rim; Observed accumulated black debris on pizza pans; No proof of certified food handler.

Coffee Cup Family Restaurant, 1512 Lafayette Ave., (3 Critical, 1 Non-Critical) – Employee observed assembling sandwich with bare hands; Stuffed peppers observed at 112 degrees, should be 135 degrees or above; Knives in rack observed with food debris.

Jonesey’s Diner, 8775 Wabash Ave., (2 Critical, 2 Non-Critical) – Sausage gravy held at 128 degrees, should be 135 degrees or above; Meatloaf not marked with date of consumption.

Ginny’s Tavern, 2938 N. 16th, (2 Critical, 1 Non-Critical) – Food date marked for more than 7 days; Handwash sink also used to dump mop water.

Taco Bell #2180, 3636 S. U.S. Hwy. 41, (2 Critical, 1 Non-Critical) – Sour cream, guacamole, and sauces in cooling unit found at 60 degrees, should be 41 degrees or less (discarded); No chemical sanitizer measured in wiping cloths buckets.
