Boston’s the Gourmet Pizza, 5601 S. U.S. 41, (4 Critical, 5 Non-Critical) — Portioned chicken and pasta in front line cooler drawers 49-51 degrees, should be 41 degrees or below; ramekins, pizza pans, knives in rack in prep area observed with food debris; ice, coffee/tea stains, tea bag observed in handwash sink in pass-through area; spray bottle of cleaner on counter with open bag of buns, sanitizer bucket on same counter.

First Wok, 2570 Wabash Ave., (4 Critical, 4 Non-Critical) — Bean sprouts and cooked lo mein on countertop found at 49-51 degrees, prep cooler had raw chicken at 48 degrees, should be 41 degrees or below; several dirty pans, knives and utensils in facility; handwash sink does not drain properly; food observed in handwash sink.

Imperial Lanes, 400 N. Third St., (3 Critical, 4 Non-Critical) — Observed ice used for human consumption on cold plate; observed a ladder in front of front bar handwash sink; found a shirt in back bar handwash sink.

Page’s Food Market Inc., 2600 Lafayette Ave., (3 Critical, 3 Non-Critical) — Cut cantaloupe in display cooler at 50-57 degrees, potato salad in deli end cap at 48-55 degrees; handwash sink in produce department dirty.

Quizno’s, 3247 N. 21st St., (3 Critical, 0 Non-Critical) — Shift manager could not demonstrate basic food safety; cheddar cheese and chicken strips found at 51-52 degrees, should be 41 degrees or below; soda nozzles observed with debris, extension cord from floor strung through tub of clean dishes.

El Camino Real Mexican Restaurant, 1119 N. Third St., (2 Critical, 3 Non-Critical) — Large pot of cooked ground beef in walk-in at 66 degrees (discarded); blade on table-mounted can opener observed with debris.

Blondie’s Cookies Inc., 3501 S. U.S. 41, (2 Critical, 2 Non-Critical) — No sanitizer measured in wiping cloths bucket; decorating tips found in handwash sink, observed employee dumping wiping cloth water in handwash sink.

Penn Station East Coast Subs No. 182, 3642 S. U.S. 41, (2 Critical, 0 Non-Critical) — Spray bottle of cleaner not labeled; open buckets of sanitizer water for wiping cloths on counter next to lemonade and raw potato.

Ballyhoo International Inc., 900 Chestnut St., (1 Critical, 5 Non-Critical) — Observed accumulated black debris on front and back bar soda nozzles, inside of ice machine and ice bin at back bar.

Pimento Marathon, 13200 S. U.S. 41, (1 Critical, 2 Non-Critical) — Prepacked sandwiches and cut melon in cooler and Landshire sandwiches in Landshire cooler at 49-51 degrees, should be 41 degrees or below.

Thornton Oil No. 78, 2665 Indiana 46, (1 Critical, 2 Non-Critical) — Found shredded cheese at hot dog station at 57 degrees should be 41 degrees or below.

Locust Street IGA, 1140 Locust St., (1 Critical, 2 Non-Critical) — Observed bean sprouts, sliced mushrooms, and cut melons at 53-59 degrees, should be 41 degrees or below.
